Booking details now separate included items from selected add-ons

Booking records and Google Calendar descriptions now distinguish plan or wallet inclusions from add-ons selected separately, while new booking-plan rules default to paid extras.

What changed

Booking Details shows the selected plan and categorized add-ons even when freelancer claim workflow is disabled.
Plan-included, package or wallet-included, and separately selected add-ons have distinct labels in Dashboard and Google Calendar.
A new Booking Plan add-on rule starts as Paid extra, and admins see a warning before bundling an add-on that already has a configured price.
Historical booking prices and stored inclusion flags remain unchanged when plan rules are edited later.
